If you wish to make a referral to us

If you are from a doctor's office or home health agency and wish to refer a patient, the process is very easy. If the patient wants us to contact them, you can fax us the patient's demographic information with contact and insurance information, as well as the PT prescription, and we will be happy to reach out to the patient to get the process going. Or, if the patient prefers, you can give them our contact information and have them directly contact us.

Since we are not a home care agency, and our service is outpatient, there is no need to setup a home health episode, nor does the patient have to be home bound to be eligible for our service.
